Grealish was part of the Villa side who lost in the final 12 months ago, being beaten 1-0 by Fulham.
But he has enjoyed another fine season for Villa, being selected in the PFA Team of the Year, and will be looking to complete his good campaign by helping the Villans to glory under the arch.
Villa vs Derby will be live on talkSPORT on Monday and Grealish will no doubt play a prominent part in the final showdown.

Jack Grealish could be a Premier League player next season
Jack Grealish salary and transfer history
Grealish has been a one club man so far in his career but did have a season-long loan spell at Notts County in the 2013/14 season.
He signed a new deal at Villa Park in September 2018 which will tie him to the club until June 2023.
His new deal double his wages to around £40,000-a-week and also included a release clause that is understood to be ‘extremely high’.
Despite his improved contract, Grealish’s future remains to be the subject of transfer speculation.
Tottenham are known to be big fans while a number of other Premier League clubs would no doubt be keen should he become available.

Jack Grealish will be in action as Aston Villa face Derby in the Championship play-off final
Former Villa goalkeeper Mark Bosnich expects Grealish to leave the club if they miss out on promotion to the Premier League on Monday.
He told talkSPORT: “I thought he [Grealish] was going to move on after last season’s defeat to Fulham, especially with all the trouble Aston Villa had off the pitch
“The fact he stayed says a lot about him, because it would have been easy for him to move on.
“But if it happens again for the second season they may be left with no choice, both player and the club.”
